You can apply to join our Group Insurance Scheme in two ways.

ONLINE

You can apply online by registering on Philip Williams website here Group Schemes - Philip Williams. Once registered you can complete the application online.

PAPER APPLICATION

Please see below Health Declaration application form to apply to join the West Mercia Police Federation Group Insurance Scheme. 

If you are unable to agree and sign the Health Declaration application, please complete the Underwritten application form which is also below. Partner’s/Spouse’s MUST complete an Underwritten application form.

Please return the completed application form to the West Mercia Federation office either by email or post on contact details. We will then forward the application to Philip Williams. You will receive notification from us in due course as to whether the application has been accepted.
 
There is no insurance in place until cover has been confirmed.

*If you have not previously been a member of this scheme, the first 3 months are free of charge (if accepted).
