Keeping Us Informed

 It is extremely important that the Federation Office Staff have the most up to date details for you.  As Federated or Staff Members in the Philip Williams Group Insurance Scheme it is vital that you update us at the earliest opportunity.  A personal data agreement will need to be completed – you can find a copy HERE and once completed send direct to office@lincs.polfed.org where a member of the team will update your record for you.

When should I complete a form:

• If you are due to go on Parental/Maternity/Adoption Leave*
• Change your Marital Status
• Moving House
• Wanting to change your nominated Group Insurance beneficiary

*We would ask that any Officer (or Staff member) in the Group Insurance due to go on Leave makes contact with us so that we can ensure your Group Insurance cover is maintained during this period and we are also aware when to recommence your Federation Subscriptions.
